The Day of Your Scheduled Visit

Please bring the following to your appointment with us:

  • Your insurance card
  • Registration and history forms
  • A list of your current prescription and over-the-counter medications (please include dose and frequency)
  • Your medical and surgical history
  • Recent x-rays or other test results
  • Immunization record (if <18 years old)

Our Promise to Our Patients

We will try our best to make an appointment to see you the same day you call, or for the day most convenient for you.

A person, not a voice recording, will take your call to answer your questions during office hours. Our enthusiastic staff is prepared to answer every question you may have.

If you call to speak with the doctor or nurse, your call will be returned on the same day.

We use an electronic medical record which allows us to utilize the most advanced technology tools in medical care. Electronic medical records allow for fast access to your data and improved management of your records. For example, we will electronically send your prescriptions directly to the pharmacy.

After your visit, we will send a letter to your primary care physician detailing a summary of our visit, including your diagnosis, prescribed medications, and treatment plan. When you follow-up with your doctor, he or she will be informed about your visit to our office for continuity of care.

We will provide up-to-date educational materials and available resources on your condition. Our goal is to help you better understand your diagnosis and treatment for your active participation in your care.
